All athletes need a current physical on file dated after April 15, 2021 for the 2021/22 school year. If your athlete is in need of a physical in the lead up to the start of the sport season Med-1 in Holland may be a good option. They offer walk-in sports physicals for a reasonable price. We are excited to announce that Hamilton High School and Middle School are utilizing FinalForms, an online system that will help us boost compliance and safety in athletics. FinalForms allows you to complete and sign forms from your computer, tablet, or mobile device. The best news is: FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice!

A few tips to help you get started:

1. Bookmark this site or save it to your desktop on your mobile device or computer.

2. Have required information available, such as medical history/health information, insurance company policy numbers, doctor, dentist and hospital information. This will make the registration time be more efficiently completed.

3. Attached is the Parent Playbook to help you with the online registration process. Please note once you register, you will receive an email to confirm your account, create a password, and then complete the required forms.

4. You are required to verify your FinalForms information each school year and print the needed annual MHSAA physical form from your account for your MD/DO/PA/NP to perform the required physical. Your completed physical form needs to be brought to your respective Hamilton Middle School (6th-8th) OR High School (9th-12th) office following your appointment. If you have previously turned in a physical we will note it on your account.
